These are the materials that must be filed with the court in a custody proceeding when one parent agrees voluntarily to have thier parental rights suspended or revoked as to a child. This doc is three pages in Word. It includes an affidavit of the parent which must be notarized and a motion, with the affidavit attached, for the judge in the case. All parental rights modifications must be approved by the judge and presented for approval in this manner. A simple agreement between parties is insufficient.
